In brief: If you are having trouble adding a user, and receive an error message that the user "already exists" or the system is giving you an error code of some type, try viewing inactive users by selecting the eye icon/show inactive on the top right hand side of the user list. You'll notice you will have the option to reactivate the user so you can promote, change their permission level and allow them access to the system again.
